HPPH Dental Screening Program (gym hall nook)

Huron Perth Public Health’s Oral Health Team will be visiting your school on​March 25 & 26, 2024, for oral health screenings. Students in JK, SK, grades 2, 4 and 7 will be screened.  

Please see the attached Principal letter outlining the oral health screening and the Parent/Guardian notification letter advising parents we will be coming to the school. Please provide the Parent/Guardian notification letter to all families at your school by March 8, 2024. It is mandatory for the parent/guardian notification to be sent at least 10 days before screening is scheduled to take place.

The following items are required upon arrival to complete the Oral Health Screening: 

·        Current class lists 

·        Table 

·        3 Chairs 

·        Garbage container 

Please respond to this email to confirm that the Parent/Guardian notification letter has been provided to students/families.  Thank you for your assistance in promoting oral health among your students.
